The Department of Health has released new guidance related to IR(ME)R incidents ‘when a person undergoing medical exposure is exposed to ionising radiation to an extent much greater than intended, other than those as a result of equipment defect or malfunction.’

The guidance has been jointly agreed by the English, Welsh, Scottish and Northern Ireland Health Departments.

It lays out clearly what steps must be taken to investigate the incident and gives guidelines on notifying the relevant IR(ME)R authority.
