Within the RTE data analysis, the most frequently reported RTE process code in the near miss (level 4) classification was ‘use of on-set imaging’ with 59 reports (7.8%), the document states.

The popular guest editorial feature for this issue comes from Suzanne B Evans, associate professor in the department of therapeutic radiology at Yale University School of Medicine.
