The new Safer Radiotherapy publication - Radiotherapy Error and Near Miss Reporting: The Unseen Pathway is now live on the website and available for viewing.
The radiotherapy pathway includes a number of safety critical activities usually unseen by the patient. These relate to the safety of the infrastructure, room design, equipment, machine QA, mould room and workshop activities. This report contains an analysis of radiotherapy errors associated with the unseen pathway for a 10 year period. In addition five case studies with a study of risk, mitigations and learning from excellence are shared.
