The Public Voluntary Register of Sonographers - Application Form

Introduction

Thank you for applying for registration on the Public Voluntary Register of Sonographers. Please have the following information to hand to complete this application:

  • Workplace details;
  • Details of any current registration;
  • Qualification details; and
  • Contact details for two referees.

Please complete the application form as fully as you can. Additional enquiries may have to be made if information requested is not provided which may delay your application.

Please note that we can only accept applications from sonographers who are already in the UK (whether working or not) or are based overseas and have a firm offer of a post in the UK.

GDPR Statement

We, the Society and College of Radiographers (SCoR), ask for the details in this online form to administer and manage the registrant's inclusion on the Public Voluntary Register of Sonographers. This information may also be used for the purposes of research and to report misconduct to other regulatory bodies. Your name, postal town, and any annotation required by the review board may be made publicly available on the Register. We may share your personal details with appointed assessors, the Health and Care Professions Council, the Nursing and Midwifery Council, the Disclosure and Barring Service, other statutory regulators and your employer. In addition, your personal details will be held on servers hosted and secured by third parties on behalf of SCoR.

Our lawful basis for processing your information is to manage your registration and fulfil our legitimate interest as a professional body.  We will retain all information you submit for the duration of either your membership with Society of Radiographers and/or the duration of the PVRS registration.

Section A - Personal details

If you are a registered medical practitioner with the General Medical Council (GMC) we are unable to accept you on to this voluntary register. The title of doctor is not statutorily protected but must not be used if working in medical practice or claiming to be registered with a licence to practice from the GMC. Section B - Current registration details (if any)

Please note: If you are registered by the UK General Medical Council as a Doctor you are not eligible to join the register.
These are the first trimester screening programme for Down's syndrome and the 18w to 20w 6d fetal anomaly scan.
DQASS: Down's Syndrome Screening Quality Statistical Support Service FMF: Fetal Medicine Foundation registration number. This is the same as the DQASS number.

Section C - Applications with standard UK ultrasound qualifications

Definition of sonographer

A healthcare professional who undertakes and reports diagnostic, screening or interventional ultrasound examinations. They will hold qualifications equivalent to a Postgraduate Certificate or Diploma in Medical Ultrasound that has been accredited by the Consortium for the Accreditation of Sonographic Education (CASE). They are either not medically qualified or hold medical qualifications but are not statutorily registered with the GMC in the UK.

  1. Evidence to support statements made with regards to qualifications, awards and dates may be required but there is no need to send these at the time of application.
  2. 'Focused' ultrasound awards accredited by CASE or other body or institution are considered 'non standard' for the purposes of the voluntary register and Section D should be completed.

Section D - Applications with alternative UK or overseas ultrasound qualifications

Please note that we can only accept applications from sonographers who are already resident in the UK and are either working as a sonographer or looking for work.

Section E - Individual Scope of practice

Your individual Scope of Practice is for our records only and will not be displayed along with your name on a public search as was previously the case. Please note that this is on a self-declaration basis. Please do not select an option unless it is a significant part of your role. For example 'Education' is intended for those who are working in universities as lecturers or have a major and distinct education role within their job descriptions. 'Clinical Applications Specialist' is intended for sonographers working for the equipment manufacturers. 'Research ' applies to those who are involved in this activity as a major part of their work and is not intended to include routine departmental audit.


Section F - References

This section is mandatory. Please provide details of two persons who may be approached to confirm your education, qualifications and current scope of practice.

Referees should be in a position to make informed comment on the scope and level of your ultrasound practice and if possible should be an employer or former employer. Friends and relatives are not acceptable. Unless there are exceptional circumstances we will not accept references from referees who are not UK based. If possible at least one referee should be registered with a statutory regulatory body in the UK such as the GMC, HCPC or NMC.

Section G - Declarations

I confirm that if I am accepted I will abide by the Standards of Proficiency and the Standards of Conduct, Performance and Ethics of the Public Voluntary Register of Sonographers.

I accept that my name and the geographical location of my workplace will appear on www.sor.org and can be searched for by members of the public.

I declare that I am actively engaged in CPD activities.

I agree to inform the Administrator of the Public Voluntary Register of Sonographers if I become subject to any disciplinary action or practice requirements imposed by a statutory regulatory body and accept that an annotation may be made on this voluntary register also.

I have read and agree to the GDPR statement on the front page of this online application form.

I understand that the College of Radiographers has the right to withdraw my name from this voluntary register and apply other sanctions as deemed necessary.

I understand that I have the right to ask for my name to removed from the register at any time.

I confirm that the information I have provided is a true and accurate record.

I have read and accept the Policy and Processes of the Public Voluntary Register of Sonographers.

 

 


Fee

New applications may carry a fee (see fees schedule).

There is no fee if you are statutorily registered and/or a member of the SCoR. If required, payment can be made by debit or credit card online - we will provide details where appropriate. Alternatively, it can also be made by cheque (or by pounds sterling bankers draft) made payable to the College of Radiographers. Your application reference number, given above, must be written on the back of the cheque or bankers draft.
